Vaccine Mandate Chicago

Chicago City Vaccine Mandate

Effective January 3rd, 2022,

The City of Chicago health order will require at venues such as the Immersive Van Gogh exhibit the following:

All Guests Age 5 And Older

Guests age 5 and older are required to show proof that they are fully vaccinated against COVID-19 when entering the Immersive Van Gogh exhibit. This means you’ve had at least two doses of a COVID-19 vaccine, or one dose of the Johnson & Johnson vaccine, two weeks prior to attending. Booster shots are not required.

Identification and Proof of Vaccination

Children ages 5 to 15 will be required to have proof of vaccination, but do NOT need to provide matching identification.

Guests over the age of 16 and over must provide identification that matches their proof of vaccination record.

Masks will remain mandatory for all guests over the age of 2, regardless of vaccination status. A negative test will not be accepted in lieu of proof of vaccination.

The City of Chicago’s announcement states that proof of vaccination means a CDC COVID-19 Vaccination Record Card or an official immunization record from the jurisdiction, state, or country where the vaccine was administered or a digital or physical photo of such a card or record, reflecting the person’s name, vaccine brand, and date administered.
